TEC statutory declaration application out of time, Collect plus letter and fine- help please!

We moved house end of Feb 2016. Royal Mail redirection arranged for 14 months.
DVLA informed of change of address to driving licence and both our cars sending off V5 forms. Didn’t notice failure of new v5 to arrive. Driving licence issued early April 2016, wife’s v5 arrived, however since sold that car so don’t have details any longer.
Received bailiff letter this week for £175, for driving in a bus lane in Reading in April 2018. Have not received any correspondence until bailiff letter.
Contacted TEC, taking out of time statutory deceleration forms to county court tomorrow for witnessed signing.
Not sure what else to do! Any help would be great!
We paid the fine as advised by TEC.
Any advice on what to write on forms would be gratefully received!
