Did they think there was a special lane open just for them?

First two got away with a bit of driver education, but it cost the third chap a few quid (s172 I presume), throw in the additional insurance premiums for the next few years and it's going to be well over a grand.

Is it complete ignorance of what the sign means or stupidity?

I think the big issue is that people don't know they are regulatory signs and thus enforceable. and they see so many both advisory and regulatory not turned off when no longer needed they ignore them

Which demonstrates the government ad spend is wasted. I hear the ad pretty much daily. Maybe it should concentrate on mentioning the penalty.

To be honest a red X over a lane is fairly self explanatory I would have thought, should you need an advert or even to read the HC to be able to surmise it means lane closed, don't use it?

Watching something similar driving along the M25 last weekend, a big red X over lane 1, lanes 2/3/4 open. Small car driven by dippy woman continues along closed lane 1 until she nearly ran into the back of the HE car blocking the lane. A friendly police officer attending to whatever disaster it was had words with her. No doubt her purse will be a bit lighter. Quite why people do it is a mystery.
